Basic Requirements
- United States or Canadian Resident
- Be a currently licensed physician (MD or DO) in the United States or Canada*

Training Requirements
Accumulate 200 points, which must be earned through a combination of points from the options below.

1. Certification in a primary board. Be certified by a primary board of the American Board of Medical Specialties (ABMS) or equivalent OR Have completed a two-year minimum relevant accredited fellowship OR Have two years of demonstrated appropriate experience and practice activity in the management of patients with lipid disorders Documentation: Copy of certificate from Board(s); Letter from Board or ACGME Fellowship indicating Board Eligibility status; Letter attesting to two years practice experience. |

2. Subspecialty certification by a board of the ABMS or equivalent OR Other relevant advanced training and/or certification Documentation: Copy of certificate from Board(s). |

3. Academic practice and/or relevant faculty appointment at an ACGME recognized institution. Documentation: Letter from Department Chair or Division Chief at the ACGME recognized academic institution(s) confirming appointment(s) and explaining relevance to lipid management. |

4. Clinical research and/or scholarly publications in the management of lipid disorders. Documentation: Highlight all relevant scholarly, peer-reviewed publications on your CV. |

5. Balance of points not obtained in 1-4, must be earned through lipid-focused continuing medical education credit obtained within the previous three years. Documentation: Attach copies of certificates of attendance fo rthe CME activities that you submit for points. The titles of the activities must be clearly identified. |
